What is my Medicaid Group Number?

I am filling out a form that asks for my insurance group and policy number. I live in Michigan, and I found the policy number on the front of my MIHealth card, but I can’t find a group number. Does anyone know where to find this?

Most likely, you don’t have one. Is it okay to leave it blank? Sure, as long as you have your ID number.

You won’t have one (like UHC) unless you have a Medicaid MCO. Very few have them on the cards, even if you do. It’s usually only discovered after the provider uses your ID to confirm your plan through their insurance verification system.
